Valley Health Achieves Perfect Score in HRSA Operational Site Visit

[January 8, 2025 – Huntington, WV]   – Valley Health Systems Inc. is proud to announce achieving a perfect score following its recent Operational Site Visit (OSV) conducted by the Health Resources and Services Administration (HRSA). This outstanding achievement confirms that Valley Health is in full compliance with all program requirements, encompassing a wide range of operational aspects including administration/governance, finance, and clinical practices.

The HRSA OSV team, made up of expert consultants in the 3 focus areas of the visit, conducted a comprehensive review over a span of two and a half days in at Valley Health – Pea Ridge and Valley Health – East Huntington. They meticulously examined 18 chapters of the compliance manual, detailing 93 subsections that require careful adherence to ensure organizational excellence. A federal representative from the Bureau of Primary Health Care, who offered technical support throughout the process.

Throughout the visit, the team praised Valley Health for its thorough preparation and the warmth and professionalism demonstrated by our staff. “We are incredibly proud of this achievement and sincerely thank every member of the Valley Health team for their hard work and dedication,” stated Dr. Mathew Weimer, President  & CEO of Valley Health.  “This extensive assessment required contributions from every individual within our organization, and it is a reflection of our collective commitment to excellence in patient care.  I witnessed firsthand the exceptional professionalism and dedication displayed by our staff throughout this process.”

In addition to a perfect score, Valley Health was recognized by the OSV team as having a Promising Practice, an honor that is not easily achieved and does not occur frequently during these visits. Specifically, the clinical consultant recognized Valley Health’s Transitional Care Management (TCM) program, which is designed to ensure safe care transitions from the hospital setting to the outpatient setting for Medicare beneficiaries. Valley Health’s visionary approach to these visits involves leadership from a team of dedicated clinical pharmacists, who work with staff, pharmacies, patients, and patients’ support systems to ensure timely follow up focused on medication safety and follow up on abnormal or outstanding tests. As a Promising Practice, Valley Health’s TCM program structure will be shared with other community health centers across the country.
