DOT Physicals – Offered at Our Milton, Wayne Apple Grove, and Harts Locations
If you’re a commercial driver needing a Department of Transportation (DOT) physical, Valley Health locations in Milton, Wayne, Apple Grove and Harts are your trusted providers. We offer both walk-in and scheduled appointments to fit your busy schedule.
What to Expect During a DOT Physical:
- Complete health screening to meet DOT certification requirements.
- Evaluation of vision, hearing, blood pressure, and overall wellness.
- Review of your medical history and current medications.
Cost:
- $101 due at the time of service.
- If you’re due for a preventive well exam, we may be able to bill insurance and complete your DOT paperwork during that visit.

Nursing School Physicals & Discounted Lab Bundles
Valley Health makes it easy for future nurses to complete the physical and lab work required for nursing school admission. These physicals and labs are self-pay services and not billed through insurance.

Physical Exam Only: $45
Healthcare Physical Panel (HCPP): $65 Includes: Hepatitis B Surface Antigen (HBSAG), Hepatitis B Surface Antibody (HBS), Hepatitis C Virus (HCV), and Hepatitis A IgM Antibody
Varicella Titer (HCPV):$20
MMR Titers (HCPMMR):$60. Includes: Measles, Mumps, and Rubella antibodies
