Services We Offer

  1. Required Immunizations and Wellness Screenings for School Aged Children: Ensure your child’s health and readiness for school by meeting specific health exam and immunization requirements. Vaccinations are necessary for starting school and throughout your child’s school-aged journey to prevent serious illnesses and maintain a healthy school environment. Wellness screenings are also essential to monitor your child’s overall health, identify potential issues early, and promote a successful academic year. Below is a guide to the mandatory exams and vaccinations needed for children in West Virginia (WV), Ohio (OH), and Kentucky (KY).
  2. Comprehensive Sports Physicals: Ensure your child is ready for the game with our thorough sports physicals. We evaluate their health, fitness level, and any conditions affecting their performance or safety. Sports physicals are not billed through insurance and are $22 at the time of visit unless the student needs a well-child exam. In this case, we will bill insurance and complete the necessary paperwork.
  3. Walk-In Services: Many of our health centers take walk-ins for sports physicals and immunizations and are open to everyone in our community. Contact your preferred health center for more information.

West Virginia (WV)

Health Exams:

  • Pre-K/Kindergarten: Well child and oral health exams
  • 2nd Grade: Well child and oral health exams
  • 7th Grade: Well child and oral health exams
  • 12th Grade: Well child and oral health exams

Immunization Requirements:

  • Pre-K/Kindergarten: DTaP, IPV, MMR, Varicella, Hepatitis B
  • 7th Grade: Tdap, Meningococcal
  • 12th Grade: Second dose of Meningococcal

Ohio (OH)

Health Exams:

  • School Entry: Well child exam

Immunization Requirements:

  • Pre-K/Kindergarten: DTaP, IPV, MMR, Varicella, Hepatitis B
  • 7th Grade: Tdap, Meningococcal
  • 12th Grade: Second dose of Meningococcal

Kentucky (KY)
