West Virginia (WV)

  • Pre-K/Kindergarten: DTaP, IPV, MMR, Varicella, Hepatitis B
  • 7th Grade: Tdap, Meningococcal
  • 12th Grade: Second dose of Meningococcal
  • HPV: Recommended starting at age 11 or 12

Ohio (OH)

Immunization Requirements:
