Diabetes Education Program

The Valley Health Diabetes Education Program is designed with a patient-centered approach to help you create a personalized plan that meets your unique needs and preferences for managing your diabetes. This program offers individual visits with our team of Certified Diabetes Care and Education Specialists (CDCES), who are experienced health professionals specializing in diabetes care and education, to help provide you with the knowledge, skills, and decision-making ability necessary to tailor an education plan to meet your specific needs to manage your diabetes.

A referral from your healthcare provider is required to participate in our Diabetes Education Program.

Diabetes Self-Management Education

Our diabetes services cover a broad range of diabetes self-management education topics, including:

  • Diabetes basics

  • Being active
  • Taking medication
  • Continuous glucose monitoring (CGM)

  • Reducing risks of diabetes complications
  • Healthy coping skills

The Valley Health Diabetes Education Program has been granted Accreditation by the Association of Diabetes Care & Education Specialists for Quality Diabetes Self-Management Education and Support.

The insurance cost of diabetes self-management education and support (DSMES) varies, but many plans cover diabetes education. If you are unsure of your coverage, please contact your insurance company directly.

4. Diabetes Medication Training – Our team will work with you and your medical team to provide insulin training and education about your medications that affect your diabetes.
