Diabetes Education Program

The Valley Health Diabetes Education Program is designed to help provide you with the knowledge, skills, and decision-making ability necessary to manage your diabetes. This program offers one-on-one visits with our team of Certified Diabetes Care and Education Specialists (CDCES), who are experienced healthcare professionals. They will help you design a personalized diabetes management plan to include the tools and support you need to improve your health and reduce your risk of diabetes complications.

A referral from your healthcare provider is required to participate in our Diabetes Education Program. Download our referral form.

Diabetes Self-Management Education

Our diabetes services cover a broad range of diabetes self-management education topics, including:

  • Diabetes basics
  • Healthy eating
  • Being active
  • Taking medication
  • Monitoring
  • Goal-setting/Problem-solving
  • Reducing risks of diabetes complications
  • Healthy coping skills

The Valley Health Diabetes Education Program has been granted Accreditation by the Association of Diabetes Care & Education Specialists for Quality Diabetes Self-Management Education and Support.

The insurance cost of diabetes self-management education and support (DSMES) varies, but many plans cover diabetes education. If you are unsure of your coverage, please contact your insurance company directly.
